Locations
360SDA provides 24/7 SIL and has access to NDIS housing solutions including SDA homes, apartments, and villas with different levels of accessibility and support.
We serve the entire Perth metropolitan and Peel areas, including suburbs such as Armadale, Baldivis, Ballajura, Erskine, Gosnells, Maddington, Meadow Springs, Midvale, Rockingham, Sinagra, Thornlie, Treeby, Wanneroo, Willetton, and Wilson.
We are planning expansion into remote areas, including Kalgoorlie and Northam.

Supported Independent Living (SIL)
Supported Independent Living (SIL) provides assistance and supervision with daily tasks to help you live as independently as possible while developing essential skills. This support can be offered in your own home or within shared accommodation settings.
At 360SDA, our SIL services include:
- Daily Living Assistance: Assistance with showering, dressing, meal preparation, and medication management.
- Life Skills Development: Focus on developing the skills of each individual to live as autonomously as possible.
- High-Intensity Support: Specialised care for complex needs such as tracheostomy care and bowel management, provided by our Registered Nurses and Enrolled Nurses.
- Community Integration: Assistance with navigating life transitions, connecting with community resources, and participating in social activities.
- Transport & Travel: Help with planning commutes, navigating public transportation, and exploring Perth’s communities.
We ensure that comprehensive training is provided to our staff, including mental health, medication administration and cooking courses, to ensure high-quality care and support.
Specialist Disability Accommodation (SDA)
Specialist Disability Accommodation (SDA) is housing specifically designed or modified to cater to individuals with extreme functional impairments or high support needs. It offers enhanced accessibility to ensure comfortable and safe living environments.
360SDA offers access to various SDA housing options designed to meet the unique needs of participants across different categories:
Improved Liveability: These homes are designed to enhance comfort and safety for individuals with sensory, intellectual, or cognitive impairments. They feature better physical access, easy-to-distinguish walls and floors, open living areas for easier support, and minimal stairs to promote independence.
Fully Accessible: Ideal for individuals with significant physical challenges, including those who use manual or powered wheelchairs. These homes feature wide doorways, accessible bathrooms, and step-free access throughout to ensure seamless mobility.
High Physical Support: These accommodations offer advanced physical access for individuals requiring extensive support. Features may include provisions for ceiling hoists, backup power supplies, home automation, and communication technologies.
Additional features such as assistive technologies, adjustable kitchen benchtops, drawer dishwashers, and pull-out pantries are available to support daily living activities and enhance safety.
We have access to various dwelling types to suit different preferences and needs, including houses, villas/units, group homes, and apartments.
SDA properties are thoughtfully designed with features and amenities to enhance your living experience:
Convenient Location: Properties are strategically located near hospitals, general practitioners, parks, shops, and other essential services, providing easy access to healthcare, recreation, and daily conveniences.
Outdoor Spaces: Many properties feature gardens or alfresco areas, offering inviting spaces for relaxation, outdoor activities, and social gatherings.
Accessibility and Furnishings: Homes are fully furnished with accessibility in mind, including wide doorways and accessible kitchens and bathrooms to promote independence and comfort.
Private Rooms: Each participant enjoys a private room and an assisted bathroom, ensuring a comfortable and secure living environment.
360SDA does not own SDA properties. Instead, we partner with a network of trusted SDA providers to offer access to high-quality accommodations. We collaborate closely with these providers to help you find the most suitable housing tailored to your unique needs and preferences. Our role is to coordinate and deliver the necessary support services, ensuring you have a comfortable and well-supported living experience in your chosen SDA accommodation.
In-Home Care
Our In-Home Care services are designed to provide personalized support in the comfort of your home. We offer:
- Personal Care Assistance: Support with bathing, dressing, grooming, and hygiene.
- Household Tasks: Help with cleaning, laundry, meal preparation, and home maintenance.
- Health Management: Medication reminders, monitoring, and coordination with healthcare providers.
- Social Support: Assistance with social activities and community engagement.
- Flexible Scheduling: Services are available at times that best suit your needs.
We customize our in-home care to meet your unique preferences and requirements, fostering independence and enhancing well-being.
Transitioning from Aged Care to NDIS
In Western Australia, approximately 600 young individuals currently residing in aged care have the chance to transition to the National Disability Insurance Scheme (NDIS). Adelphi Living is dedicated to facilitating this transition, offering a fresh start focused on independence, personalized support, and dignity.
We recognize the unique challenges faced by young people in aged care and are committed to empowering them with a smooth and supportive transition to a more independent and fulfilling life.

Respite Care
Respite Care offers temporary support and accommodation for participants, providing primary caregivers with a much-needed break. It helps both you and your caregiver recharge and maintain a healthy, balanced lifestyle.
NDIS Short Term Accommodation (STA) typically covers stays of up to 14 days at a time with Adelphi Living. However, the duration can be flexible based on individual needs and plan funding.
NDIS Medium Term Accommodation (MTA) supports participants who need temporary housing for periods longer than STA, generally up to 90 days. This is often used while awaiting permanent accommodation solutions such as SDA.
NDIS and Funding
The NDIS is an Australian Government initiative that provides funding and support to individuals with permanent and significant disabilities. It aims to help participants achieve their goals, enhance their independence, and improve their quality of life.
You can apply for NDIS support if you are under 65 years of age at the time of your application.
However, people with disabilities living in residential aged care under the age of 65 can apply for NDIS registration now and continue with NDIS services and funding beyond this age. Visit our Transitioning from Aged Care to NDIS page for more information.
To become an NDIS participant, you must apply and be assessed for eligibility. You can apply for the NDIS online, by phone, or by completing a paper form. Once approved, you can begin planning your support needs with your NDIS planner.

When choosing an NDIS provider, consider their experience and expertise in providing services to individuals with disabilities. NDIS plans are typically reviewed annually. However, you can request a review at any time if your circumstances or support needs change.
Funding for SIL and SDA is determined based on individual assessments of your support needs during the planning process. Your NDIS planner will work with you to identify the appropriate level of funding
Staff and Support Workers
Our support workers are committed to providing exceptional care. All staff members are required to have:
- NDIS Worker Screening Clearance
- First Aid & CPR Certifications
Additionally, our support workers may have specialised training in:
- Medication Assistance Certification
- Manual Handling Certification
- Insulin Administration Training
- PEG Feeding Training
For Registered or Enrolled Nurses, we require current AHPRA (Australian Health Practitioner Regulation Agency) registrations.

We take great care in ensuring compatibility among participants in shared accommodations. To support a smooth transition, we arrange meet-and-greet sessions between new participants and existing residents, along with their families or representatives. Our person-centered approach ensures that all care needs are met by our dedicated Support Workers, House Coordinators, and Registered Nurses/Enrolled Nurses.
An NDIS Support Coordinator assists participants in understanding their NDIS plan, accessing support services, and managing their care. They help participants make the most of their funded supports to achieve their goals.
